Mail-in application

If you wish to mail in your membership application, please follow the following steps:

  • Click on the "Membership form" link below to display the form
  • Select the "printer friendly" page option and print the form
  • Fill the form out
  • Attach your check for the appropriate amount ($15 per person/$30 per family)
  • Mail the form and check to:  Nebraska Chapter APDA, 501 N. 87 Street, Suite 207, Omaha, NE  68114.
